Prevalence and Characteristics of Sexual Dysfunction in 57 HIV-Positive Women

Background: There is very little knowledge on prevalence of sexual dysfunction in HIV-infected women as compared to men where it is known to be high. Methods: We conducted a transversal study in 57 HIV-infected patients matched by age with 30 HIV negative women as controls. Study was based on a 160-items self-administered questionnaire (3 sections of the DSFI, FSFI and open questions). Statistical analysis was performed on SPSS software (Kruskall-Wallis for non parametric variables). Results: 57 patients were include. Mean CD4 count was 517/mm3, mean age was 38 years, 88% were on HAART. 33% reported the absence of a regular sexual partner. 19.3% reported sexual dysfunction before HIV, 35.1% after, and 51.8% after the onset of treatment. Decrease or loss of libido, orgasm perturbations and dyspareunia were scored as very severe/severe in 53.5%, 45% and 37.5% respectively. There was no statistical difference between mean scores of FSFI section, though 12 items showed strong statitical difference between patients and controls.A responsible event for sexual dysfunction was reported as HIV infection in 67%, body image alteration in 35% and the onset of treatment in 29% of the cases. Conclusions: There is a high prevalence of sexual dysfunction in this group of women with good immunological status. As sexuality is a major part of quality of life, and as it may influence adherence and sexual risk behaviors,physicians should be able to diagnose and take care of such perturbations.
